We are looking for participants for our Diabetes Education program. This group program includes a one-on-one session specific to you to discuss your metrics, concerns, goals, etc., tailored to your situation.
Spots are limited and eligible participants will be accepted on a first-come, first-served basis.
It IS POSSIBLE to take control of Type 2 Diabetes and not let it overcome your body or your life.
Call us if you're interested in participating in our Diabetes Self-Management Education & Support (DSMES) program and we will check eligibility.
